Gut stack checklist: fiber first, then a 4 to 8 week probiotic trial

A short, sourced routine. Get your fiber right first. Then, if you want, try one probiotic chosen from what its label shows, and judge it with your health care provider.

In short

Start with fiber from food. MedlinePlus puts the target for adults at 21 to 38 grams a day, while the average American eats about 16, and says to add fiber slowly to limit gas. If you then try a probiotic, choose one whose label names the strains and lists CFU through the expiration date, and ask your health care provider which one, what dose and how long. NIH notes there are no official recommendations for probiotic use by healthy people.

About the "4 to 8 week" in the title. It is a working figure. No source we opened gives a trial length, so the number stays out of the checklist until a source or a Physician's Choice approved line exists.

Step 1: fiber first

MedlinePlus says adults should eat 21 to 38 grams of fiber each day. On average, Americans eat about 16. Food is the first place to close that gap. For prebiotic fiber in particular, our prebiotic foods list gives the grams of inulin in chicory root, garlic, onions and other foods. If you are weighing a prebiotic supplement instead, our guide to prebiotic supplements lists the grams trials used for each type.

Two cautions from the same sources. Increasing fiber too quickly can lead to gas, bloating and cramps, so add it slowly. And water helps fiber pass through the digestive system. (MedlinePlus)

If food alone falls short, a fiber supplement can fill part of the gap. One label example is Easy Mix Fiber: 6 g of soluble fiber per heaping spoonful, from soluble corn fiber and guar fiber.

Step 2: a probiotic trial, if you want one

This step is optional. NCCIH notes that strong evidence for most uses of probiotics is lacking, and the AGA's 2020 guideline found the evidence for probiotics in most digestive conditions greatly lacking. NIH's Office of Dietary Supplements says there are no official recommendations for probiotic use by healthy people, and suggests asking a health care provider which probiotic to choose, what dose to take and how long to use it.

Wondering whether the probiotic needs a prebiotic beside it? Our guide Do I need to take a prebiotic with my probiotic? sets out what ISAPP, NIH and Cleveland Clinic say, and how prebiotic doses on labels compare with the grams used in studies. If the two words still blur together, start with prebiotic vs probiotic: what is the difference?

If you go ahead, the label tells you most of what to check. In healthy people, probiotics may cause gas, but they rarely cause infections or other health problems. (NIH ODS)

What a gut stack does not need

Many online stack lists add five or more products on top of fiber and a probiotic. More products do not add up to more evidence: the reference sources above describe limited evidence even for probiotics on their own. A short stack is also easier to judge. If something changes, you know what changed.

Physician's Choice, which publishes this site, makes one product for each step. They are shown as label examples, not as the only choice.
